- Your Social Insurance Number
- Your Permanent Code
- The codes for your educational institution and program
- Your course dates
- Your 2022 income (If the year has not ended, provide an accurate estimate of your income)

Upload your documents to Your Student Financial Assistance File. They will be processed faster if you submit them online rather than by mail.
Other way to submit your document
Save the form to your computer to fill it out. Once filled out, you must mail it to the address shown on the form itself.
