Schedule 2 – Student’s Parents or Sponsor

This form is to be used for applications relating to the September 1, 2022 – August 31, 2023 award year.

You must fill out this form if you are the parent(s) or sponsor of a student or future student who is applying for a loan for part-time studies.

Before you begin, make sure you have the following information in hand:

  • Your Social Insurance Number
  • Your income
  • The student’s Permanent Code

If the parents live together, the declaration must include both their incomes. If they do not live together, only the parent with whom the student lives or was living is required to submit a declaration.

If you do not have a Social Insurance Number or do not know the student’s Permanent Code, please refer to the Application for Financial Assistance, Loans for Part-Time Studies guide.

You need Adobe Reader version 8 or later to fill out this form once you have saved it to your computer. After the form is filled out, print it, then sign and mail it to the address shown on the form itself.
