You need to submit this form if you are pursuing studies at an educational institution outside Québec.
We must receive this form at the beginning of the academic year and may be required to receive a new form during the year to confirm your attendance.
Confirmation of your main educational activity for each of the 12 months of the award year (September-August) that makes it possible to validate the following:
- Your attendance in the preceding months
- Your studies in the coming months
Two forms will be required if the student is enrolled in two different programs or at two different educational institutions during the same award year.
This form must be sign by an authorized person at your educational institution.
