Schedule 1 – Declaration of Student’s Spouse

This form is to be used for applications relating to the September 1, 2017–August 31, 2018 award year.

You must fill out this form if you are the spouse of a student or future student who is applying for a loan for part-time studies.

Before you begin, make sure you have the following information in hand:

  • Your Social Insurance Number
  • Your income
  • The student’s Permanent Code

If you do not have a Social Insurance Number or do not know the student’s Permanent Code, please refer to the Application for Financial Assistance, Loans for Part-Time Studies guide.
