Declaration of Spouse

If you are the spouse of a student and you are required to contribute toward the cost of his or her education, you must send us the Declaration of Spouse form.

By Internet

You can fill out the Declaration of Spouse form. Simply click On-Line Access to Your File.

By mail

Download and print the Declaration of Spouse (PDF 160 ko) form, fill it out and mail it to us. You can also obtain the form from the financial assistance office at your spouse’s educational institution.

Notice of change required

If the information provided is no longer accurate because your circumstances changed during the year, you must notify us. These changes could affect the amount awarded to your spouse.

By Internet

You can notify us by filling out the Declaration of Change form. Simply click On-Line Access to Your File.

By mail

You can notify us by sending a letter. Remember to specify the student's permanent code.

Drop in income of 10% compared with the previous year

If your income for the current award year drops by 10% or more compared with the previous calendar year, you must fill out and send us the Declaration of financial resources - Spouse (PDF 34 ko). Your income for the current award year will then be considered to determine your contribution.

You will be informed whenever a change is recorded in your file. If you provided an e-mail address, an e-mail will be sent to you asking you to consult the On‑Line Access to Your File section for information about the change in question. If you did not provide an e-mail address, a letter will be mailed to you.
