
If you are the parent of a student and are required to contribute toward the cost of his or her education, you must send us the Declaration of Father or Declaration of Mother form. Both the father and the mother must fill out a form. However, if one of the parents has been awarded physical custody, only that person is required to fill out the appropriate form. Please note that a parent who is required to fill out a form need only fill out one form, regardless of the number of dependent children who apply for student financial assistance.
In the same vein, if you are the sponsor of a student who has applied for financial assistance and you are required to contribute toward the cost of his or her education, you must send us the Declaration of Father or the Declaration of Mother form, as the case may be.

69 ko) form, as the case may be, fill it out and mail it to us. You can also obtain the form from the financial assistance office at the student’s educational institution.
You must fill out the Declaration of Father or Declaration of Mother form on the basis of the information at your disposal at the time of the signature.
If the information provided is no longer accurate due to a change in your circumstances during the year, you must notify us. These changes could affect the amount of assistance awarded to the student.
You can notify us by filling out the Declaration of Change form. Simply click On-Line Access to Your File.
You can notify us by sending a letter. Remember to specify the student's permanent code.

If your income for the current calendar year drops by 10% or more compared with the previous calendar year, you must fill out and send us one or both of the following forms, depending on your family situation: Declaration of income drop of at least 10% for 2009 — Father (
38 ko) and Declaration of income drop of at least 10% for 2009 — Mother (
38 ko).
If both father and mother are required to contribute toward the cost of their child’s education, they must each fill out a form, even if only one parent’s income drops. However, if physical custody of the child is awarded to one parent, he or she is the only parent required to fill out a form. To determine his or her contribution, the parent’s income for the current award year is considered.
If there is a difference between the child assistance and Canada Child Tax Benefit payments you actually received and those considered income in the assessment of your contribution, you must send us a file correction request.
You will be informed whenever a change is recorded in your file. If you provided an e-mail address, an e-mail will be sent to you asking you to consult your online file for information about the change in question. If you did not provide an e-mail address, a letter will be mailed to you.
